What Medical Evidence Curtin University Accepts for an Assessment Extension
If you are already enrolled at Curtin, special consideration is not your process. Here's what you actually need, and what your evidence must look like.

At Curtin, special consideration is an admissions process for people applying to the University. Current students who are unwell before an assessment apply for an Assessment Extension through OASIS instead. Curtin accepts a signed statement, such as a medical certificate, on official letterhead from a medical practitioner or another health professional registered with the relevant National Board. Documents must be in English. No Curtin practitioner form is required for the general assessment extension route.
Curtin uses "special consideration" to mean something other universities do not
This is the single most common mistake Curtin students make. At most Australian universities, special consideration is what you apply for when illness affects an assessment. At Curtin, Admissions handles special consideration and offers it only to people new to the University. Curtin's special consideration guidelines state plainly that current students are not eligible to apply through that process and direct them to Student Wellbeing Advisors for advice.
If you are already enrolled and were unwell before an assessment, you need an Assessment Extension, governed by the Assessment and Student Progression Manual. Your student medical certificate for Curtin attaches to that application, not to a special consideration form.
Evidence goes through OASIS with the application
Curtin students lodge through OASIS and attach the supporting evidence to the request. There is no separate email submission and no paper lodgement for the standard route.
What counts as valid supporting documentation
Curtin accepts a signed statement on relevant official letterhead from a medical practitioner or another approved health professional registered with the relevant National Board. The supporting documents guidance also lists signed statements on letterhead from Curtin's Psychological and Counselling Services or AccessAbility Services, explaining how the circumstances affected your ability to submit by the due date. Documents must be in English.
You do not need a Curtin practitioner form
Unlike Western Sydney, Macquarie or Swinburne, Curtin does not require its own form completed by your GP for the general assessment extension process. A properly written certificate on letterhead is enough. If you are booking a certificate, you can skip the university form option.
Apply before the assessment where you can
Applying ahead of the due date is the strongest position. A late application can still be considered where a reasonable explanation and suitable evidence support it, but the burden shifts to you to explain the delay.

Curtin runs admissions special consideration and student assessment extensions as separate processes with separate criteria, and deferred assessment is a third pathway. If you are unsure which applies, check the current Curtin special consideration page or speak with a Student Wellbeing Advisor before lodging, because applying through the wrong one costs you time you may not have.
