What Is a Fitness for Work Certificate?
A fitness for work certificate documents a GP's assessment of whether you can perform your work duties safely and, where relevant, whether temporary restrictions or modified duties are needed.
It answers a different question from a sick leave certificate. Sick leave documents a period when you are medically unfit for work; fitness for work focuses on your current capacity to return, continue working or perform particular duties safely.
The assessment should therefore be matched to your actual role. A restriction such as avoiding heavy lifting, limiting prolonged standing or changing work hours is only useful when it relates to the tasks the employee is expected to perform.
Capacity, not just diagnosis
The GP considers what you can safely do, what remains temporarily limited and whether another assessment is required before full duties resume. Post-absence clearance, modified duties and role-specific medical clearance can require different assessments.
A Fitness Assessment Can Have Different Outcomes
Fit for normal duties
The assessment supports returning to the usual role without specified temporary restrictions.
Fit with modified duties
The GP may document measurable temporary limits where some work remains safe, such as changes to tasks, hours or work pacing.
Not appropriate to clear remotely
Some safety-critical duties, complex injuries or examination-dependent questions may require in-person, workplace-specific or specialist assessment before clearance can be given.
Check Whether Your Clearance Can Be Completed by Telehealth
A fitness-for-work certificate isn't a one-size-fits-all test. The GP needs to know what the employer, regulator, airline, training provider or other organisation is asking the person to be cleared for.
Telehealth can be suitable where the decision can be made from the history, current symptoms, available records and a real-time consultation. It may not be suitable where the requesting organisation requires a physical examination, vision or hearing testing, spirometry, functional capacity testing, drug and alcohol screening, or another test that cannot be performed remotely.
Upload the request before your consult
Upload any employer or organisation form before the consultation. The GP can then assess the actual duties and decide whether the appropriate outcome is unrestricted fitness, temporary restrictions, modified duties, further assessment, or no clearance at that time.
You can also review what OMC Health cannot issue or assess remotely before booking.
Who May Need a Fitness for Work Certificate?
A fitness assessment may be requested after extended illness, injury or surgery, for modified duties, before some roles or activities, or where an employer or other organisation needs a role-specific capacity decision.
The consultation is conducted in real time by phone or video. The GP may require video or recommend in-person assessment where visual or physical examination is necessary.
Clinical decisions remain subject to professional judgement and the standards described in the Medical Board of Australia's Good medical practice code.
Return-to-Work
After injury, surgery or extended sick leave where current capacity needs to be documented.
Pre-Employment
Where a new role requires a defined fitness assessment and the requested components can be assessed appropriately.
Medical Clearance
For activities, study, placement or other organisation-specific clearance requests.
Modified Duties
Documenting measurable restrictions where some work remains clinically appropriate.
Fit to Fly
Airline-specific medical clearance where the required assessment can be completed within the consultation scope.
Safety-Critical Roles
Roles involving driving, machinery or other safety-sensitive duties may require specific standards, testing or in-person assessment.
Fitness Status Explained
The assessment supports performing the normal duties considered during the consultation.
Work may be appropriate with specified temporary restrictions or modified duties.
The GP may be unable to support return at that time or may require additional assessment before making a clearance decision.
Driving clearance can have separate regulatory requirements. Australia's Assessing Fitness to Drive standards set national medical standards, while licensing authorities such as Transport for NSW specify forms and assessment processes.

What the GP Assesses During the Consultation
The GP assesses your current functional capacity against the relevant demands of your role, including the symptoms, history, recovery progress and task-specific information available during the consultation.
The consultation is conducted in real time by phone or video. The GP may require video or recommend in-person assessment where visual or physical examination is necessary.
What the GP reviews
- ✓Your symptoms and relevant medical history
- ✓How the condition affects your work capacity
- ✓The physical, cognitive and safety demands of your role
- ✓Recovery progress, treatment and available records
- ✓Appropriate duties, restrictions or need for further assessment
The GP determines what capacity outcome can be clinically supported from the assessment and available information.
Your Certificate May Include
Your certificate may document full clearance, modified duties, specific limits or review requirements based on the GP's clinical assessment.
The assessment supports return to the usual duties considered by the GP.
You may return with stated work restrictions or limits.
The certificate may specify safe limits for tasks, hours or duties.
Task-specific duties may be identified where clinically appropriate.
A review date may be included where capacity needs reassessment.
Clear capacity and restriction wording helps the worker and employer understand the GP's assessed work limits without treating the certificate as a substitute for workplace risk assessment.
What Your Certificate Says About Capacity and Restrictions
A fitness-for-work certificate should make the capacity decision clear: unrestricted fitness, temporary restrictions, modified duties, review date or a need for further assessment. It should not be presented as a generic certificate-validity checklist.
An OMC Health fitness for work certificate also records the assessment and practitioner details needed to identify the issued document. Where restrictions apply, they should describe the relevant functional limit clearly enough to support practical work planning without unnecessarily disclosing the underlying diagnosis.
Restrictions should be practical and measurable
Where clinically appropriate, the document may describe limits such as lifting tolerance, standing duration, work hours or another task-specific restriction rather than using a vague "light duties" label.
Details recorded on the issued certificate
The document records the assessed capacity outcome together with the details needed to identify the practitioner and certificate.
Assessment date
Date of the GP consultation.
Capacity outcome
Unrestricted fitness, restricted capacity or another clinically supported outcome.
Restrictions or modified duties
Relevant functional limits or conditions where restrictions are clinically required.
Practitioner details
Issuing GP's name and AHPRA registration details.
Issuing practice details
Details identifying the medical practice responsible for the issued document.
Signature and verification
Practitioner signature plus QR code or unique verification details.
The certificate documents the capacity outcome supported by the GP's assessment. It does not guarantee that every role, regulated activity or safety-critical duty can be cleared through telehealth.

Return-to-Work Certificates
Return-to-work clearance focuses on whether normal duties, graduated hours or temporary restrictions are clinically appropriate after illness or injury. Safe Work Australia's National Return to Work Strategy provides the national framework for safe and durable return to work. A common recovery scenario is covered in our concussion return-to-work guide.
Return-to-work certificate →Modified Duties Certificates
A modified duties certificate documents specific functional restrictions where some work remains appropriate. A condition-specific example is covered in our sciatica modified-duties guide.
How modified duties certificates work →Pre-Employment Medical Certificates
Pre-employment assessments are matched to the inherent requirements of a role. Physical testing, hearing checks, drug and alcohol screening or other components may require an in-person provider.
Pre-employment medical certificate →Medical Clearance Certificates
Medical clearance requests for activities, placements, study or other organisations depend on the requesting body's checklist and may require records, examination or testing beyond a standard telehealth assessment.
Medical clearance certificate →Fit to Fly Certificates
Airlines set their own medical clearance requirements. The consultation is conducted in real time by phone or video, and the GP may require video or recommend in-person assessment where visual or physical examination is necessary.
Fit to fly certificate →Fitness to Drive Assessments
Driver medical assessments follow national and state licensing standards and can require vision, physical or other testing that cannot be completed remotely. This pathway explains the scope before you arrange the required assessment.

Frequently asked questions
View all FAQsIf the GP determines a certificate is clinically appropriate, it is digitally issued and emailed the same day as your consultation. Video consultation is preferred so the GP can directly observe functional movement and presentation.
Yes. The GP can document specific restrictions, no lifting above a specified weight, sedentary duties only, limited hours, no driving. Restrictions are stated in measurable terms, not vague wording like 'light duties'.
No. A sick leave certificate documents an absence from work. A fitness for work certificate documents clearance or capacity to work, either fully or with restrictions; and is often required before returning after extended leave.
An online fitness for work certificate may be accepted where it provides the information your employer requires about your capacity to work or any relevant restrictions. Some roles, particularly safety-critical or specialised work, may require an in-person occupational health or specialist assessment instead.
Yes. OMC Health GPs can assess fitness for the specific demands of a role before you start a new job. The assessment focuses on your capacity relative to role requirements, not your full medical history.
