WSU asks for its form, not just any certificate

Most universities accept a standard certificate. WSU is an exception, and that difference catches students out. The University maintains a medical certificate form and asks the practitioner to complete it. If you provide a practitioner's own certificate instead, the medical information must address the same points the University's form asks for, and it must be on official letterhead. A student medical certificate issued without regard to those points can still be declined.

The practical move is to download WSU's form before you book, then attach it during booking so the GP has it in front of them at the consultation. OMC supports this as a booking option.

Independent evidence is the standard, and a personal account is not evidence

WSU's supporting documentation requirements are explicit that a letter you write yourself explaining your circumstances does not meet the bar. Evidence must be independent, signed and dated, and, where relevant, on official letterhead. Documents in a language other than English need a certified translation.

A statutory declaration is a last resort, not an alternative

WSU will consider a statutory declaration only where no other supporting document is available. It is not a substitute for a medical certificate when a medical certificate could reasonably have been obtained.

Documentation signed by a relative is not normally accepted

Even where the relative is a registered practitioner. This trips up students whose parent or sibling works in health.

Deferred exams run on a separate and tighter clock

Deferred exam applications follow their own process, with their own form and deadline tied to the scheduled exam, not a general post-assessment window. Check WSU's current deferred exams guidance rather than assuming the special consideration timeframe carries over.

Keep the original, because WSU may check it

WSU may contact you to verify submitted documents within a year of submission. Keep the original PDF and any certificate reference number until your result is final.

Before you apply, check that WSU has not changed the form

WSU updates its student forms page between academic years, and the deferred exam route has its own separate form and deadline. Download the current version of the medical certificate form from WSU's current student forms page on the day you apply rather than reusing a copy from a previous session.
